Title: A Randomised Controlled Multicentre Study evaluating changes in Cardiovascular Risk Factors in Smokers with type 2 Diabetes Mellitus after switching from Tobacco Cigarettes to Combustion-Free Nicotine Delivery Systems: DIASMOKE 2.0 Study

This study aims to test the hypothesis that avoiding exposure to cigarette smoke toxicants may translate to measurable improvement in cardiovascular risk factors and functional parameters when type 2 diabetes mellitus (T2DM) patients who smoke switch to using C-F NDS compared with T2DM patients who continue to smoke conventional tobacco products.
Participants will be randomized (1:2 ratio) to either continue to smoke commercially manufactured cigarettes (Study Arm A) or combustion-free nicotine delivery systems (C-F NDS) (Study Arm B).
Before randomization, all smokers will be reminded of the risks associated with smoking and will be offered a free smoking cessation program according to standard local guidelines and depending on the local availability of antismoking services. Those who decline the invitation will be eligible for recruitment into the RCT.
Following a screening visit, participants will be invited to five study visits over a 24 month period, and will undergo a range of measurements and blood tests.

GENESIS Study: A 12-month randomized, double-blind, controlled, international multicentre trial comparing changes in Cigarette consumption after switching to high or low nicotine strength E-cigarettes In smokers with Schizophrenia spectrum disorders.

Although smoking it is rewarding for you, it is bad for your general health. It is also particularly harmful for people living with some mental health conditions. For example, smoking is associated with more depressive symptoms, increased stress, enhanced psychotic symptoms and poor response to antipsychotic treatments.

By stopping smoking you could experience an overall improvement of psychiatric symptoms with less anxiety and depression with an overall improvement of your psychological quality of life. Moreover, you might be able to reduce the level of medications you’re taking.

Considering what has been said so far, quitting smoking will help manage your mental health condition better and contrast the development and the progression of smoking related diseases. No matter how much or how long you have smoked, quitting always helps your health in many ways.

Quitting may not be easy, however. It has been suggested that for people not intending or not able to quit smoking, it is easier to substitute conventional tobacco cigarettes with combustion-free nicotine delivery systems (known as e-cigarettes) to achieve potential health gains.

GENESIS Study is investigating whether switching to these combustion-free nicotine delivery alternatives reduces your need to smoke tobacco cigarettes.

In particular, the results of the study may help us to understand if switching to e-cigarette can accomplish high quit and reduction rates that persist long-term in people with mental health problems.

This study is exploring sensitive measurements of early signs of tobacco harm. It is well established that tobacco smoking causes damage to your lungs but there is no easy diagnostic test to quantify. The results of this study may help us to identify sensitive markers using CT scans and respiratory tests. These measurements will be linked with blood, urine and breath biomarkers. We hope, in future, simple blood, breath or urine test can be used instead of CT scans and respiratory tests to quantify lung damage caused by tobacco smoking.
